34th ACSIC Training Program
Participant Consent Form
This consent form (the “Consent Form”) applies to participants registering for the upcoming 34th ACSICTraining Program, hosted by Credit Guarantee Corporation of Cambodia (CGCC) in Siem Reap province, Kingdom of Cambodia (the “Program”).
By submitting the digital registration form, the participant acknowledges that they have read, understood, and agreed to the terms and conditions set out below.
- Declaration of Information Accuracy
The participant confirms that all information and/or documents provided in the registration form is true, accurate, complete, and up to date. The participant acknowledges and agrees that such information and/or documents may be used by the host organization solely for the purposes related to the planning, coordination, communication, and administrative arrangements of the Program.
- Use and Sharing of Training Materials
The participant acknowledges and agrees that any materials, presentations, documents, or other content shared or presented during the Program may be collected, used, reproduced, and shared by the host organization for the purposes of knowledge, dissemination, capacity building, reporting, and information sharing among members of the ACSIC network and other relevant stakeholders. Such use shall be non-commercial and aligned with the objectives of the Program.
- Photo, Video, and Media Consent
The participant grants permission to the host organization to capture, record, use, reproduce, and publish photographs, video recordings, and other audio-visual materials taken during the Program. These materials may be used for documentation, reporting, promotional, and acknowledge-sharing purposes across various communication channels, including but not limited to official publications, websites, presentations, and social media platforms, without compensation.
- Collection and Use of Personal Data
The participant acknowledges and agrees that
personal data, including but not limited to name, contact details, organization, position, and other relevant information, will be collected and processed solely for purposes related to the organization, administration, communication, implementation, reporting, and follow-up of the Program.
- Personal Data Sharing
The participant acknowledges and agrees that the participant’s information may be shared with relevant parties involved in the organization and delivery of the Program (such as co-hosting institutions, accommodation providers, or service providers), strictly on a need-to-know basis and for Program-related purposes only.
- Data Storage and Security
The host organization will implement reasonable administrative and technical measures to protect personal data against unauthorized access, disclosure, alteration, or misuse.
- Data Retention
Personal data will be retained only for as long as necessary to fulfill the purposes of the Program and related reporting or compliance obligations, unless a longer retention period is required by applicable law and regulation.
- Participant Rights
The participant has the right to request access to, correction of, or deletion of their personal data, subject to applicable laws and the operational requirements of the Program.
- Voluntary Consent
Consent to the collection, use, and processing of personal data is provided voluntarily. By submitting the digital registration form, the participant provides explicit consent to the terms as outlined above.
